Choose how you expect settlements to be handled. For security, do not enter full bank routing or account numbers on this page. ACH banking details should be completed only through ELTA's secure payment setup process.

Choose how dispatch services will be handled while leased to ELTA Logistics LLC. S&L Dispatching is optional. You may choose S&L Dispatching at 7%, use your own dispatcher, or self-dispatch.

Complete the official IRS Form W-9 for tax and payment setup. The current IRS Form W-9 is the March 2024 revision. Open it below, fill it in, save the completed PDF to your device, then upload it in the document section.

Upload clear copies or photos of the requested documents. Accepted file types: PDF, JPG/JPEG, and PNG. Items marked “if applicable” are only required when they apply to the applicant or equipment.

ELTA should still obtain/verify company-controlled compliance items separately, such as the MVR, Clearinghouse query result, pre-employment drug-test result, and final internal compliance approval.

After completing the application and selecting your documents, continue to the ELTA Logistics Owner-Operator Lease Agreement. The lease is fillable and editable online before the final signing/submission step.
